Please remember that the appointments made on the form below are for our Sydney CBD, Baulkham Hills and Seven Hills clinic. Alternatively you can make an appointment by emailing appointments@travelvaccines.com.au

Step 1 - Select from the 3 Travel Vaccine Locations (Date field will appear upon selection)
Step 2 – Enter your Name, Phone and Email address
Step 3 – Click the calendar icon and select your preferred appointment Date (Time field will appear upon selection)
Step 4 - Select your preferred appointment Time
Step 5 – Scroll to the bottom and click Submit Request
Step 6 – You will receive a email confirmation once booking has been completed
